The game works fine, both extraction and install. If you're getting install errors on PS4; reboot (I've noticed HEN can sometimes go a bit screwy and installing can sometimes throw a C5 error) and if you're getting errors on extraction then you need to use WinRAR 5 or above, or something equivalent that supports WinRAR v5 compression.

If anyone is looking for this in the future just use ps2-fpkg v.06 It's easy.
1. get the ps2 iso
2. open ps2-fpkfg as admin
3.make a txt file name it whatever you want.
copy this into the txt file.

--vu1-no-clamping=0
--vu1-clamp-operands=1
--vu1-clamp-results=1
--gs-adaptive-frameskip=1

# Fix for graphical corruptions, require "JAK" emu.


4. in general tab load your iso and background(1280x1080) and icon(512x512 pics you want
5.. in the emulator settings tab add your txt file you made
6. hit create create fpkg
